    CLOTHING
    You will need a week’s worth of clothing including:
    • Comfortable pants/shorts
    • Cotton Shirts
    • Robe
    • Pull over house gowns
    • Dresses – buttoned down
    • Underwear
    • Bras
    • Socks
    • Pajamas
    • Shoes (tennis shoes, please no slippers)
    • Afghan/Lap blanket
    PERSONAL SUPPLIES
    • Dentures
    • Eye glasses
    • Adult briefs (available through facility & covered by Medicare & Medicaid)
    • Cosmetics
    • Razor (electric if desired)
    • Perfume or cologne
    • Personal items from home (pictures, stationery & pens, books, mementos)

    • All items must be marked.
    • Clothing labels are available through facility. An inscription appliance is available to use. Please ask housekeeping to assist you.
    • All furnishings and electrical materials must be inspected by maintenance with fire safety codes. Please notify your nurse or Social Services when bringing in these items.

    You should look for cleanliness, staff should be friendly. Look for a facility which has a specially designed and separate rehab unit.

    What is skilled nursing/sub acute rehab care?2021-08-18T18:05:03+00:00

    Sub acute care is comprehensive inpatient care designed for someone who has an acute illness, injury, or exacerbation of a disease process. It is goal oriented treatment. Sub acute care is generally more intensive than traditional nursing facility care and less than acute care it requires treatment plan for a limited time period (several days to several months), until the condition is stabilized or a predetermined treatment course is completed.

    There are no set visiting hours; however it is best to check with staff for after business hours visitation protocol.

    If I am admitted will I have papers to sign?2022-02-07T20:11:34+00:00

    Be prepared to sign admission forms. These may include a financial screening performed with our financial counselor when filing for Medicaid.  If you need help, have your representative, power of attorney, or family member available to work with us prior to your admission. If you have special directive documents, bring them with you so we may make copies for our records.

    Bring all medical insurance cards so we may verify your insurance and make sure you have coverage. Please include any commercial insurance cards/plans you have such as: Medicare Part D, Medicare A & B, Medicaid, Blue Cross, AARP, HMO, PPO’s, etc.

    Meals are served with social distancing in mind in our main dining room, rehab dining area or privately in your room. Guest meals are available at a nominal cost.

    Semi-private rooms cannot accommodate overnight visits.
